SM-04554 (Samumed, now Biosplice Therapeutics) is a topical small molecule that ACTIVATES the Wnt/β-catenin pathway – a regenerative pathway that plays a key role in the development and cycling of the hair follicle. The idea is that stimulating the Wnt pathway could trigger the formation of new hair follicles and an increase in follicle density, which is a fundamentally different approach from DHT inhibition (antiandrogens) or improving perfusion (minoxidil). CLINICAL REALITY: in the phase 2 trial, hair count increased statistically at some doses compared with vehicle, but the MAGNITUDE of the effect was MODEST and not clearly robust, and the further development status is uncertain. SM-04554-specific, peer-reviewed, confirmatory efficacy publications are scarce; the Wnt-modulator class is covered in a review article (PMID 34128760). Overall: an interesting regenerative concept, but a CLINICALLY UNPROVEN, mixed-result, investigational agent.
