What is donepezil used for?

Donepezil is a prescription medicine for the symptomatic treatment of dementia due to Alzheimer's disease, from the mild to the severe stage. It can improve memory and daily function, but it does not stop or reverse the underlying neurodegeneration.

Why is it taken once daily in the evening?

Donepezil has a half-life of about 70 hours, so a single daily dose is enough to maintain a stable blood level. Many patients take it in the evening, but if vivid dreams or sleep disturbance are bothersome, a doctor may switch dosing to the morning.

Can it be used as a cognitive enhancer in healthy people?

It is not approved for that. In healthy adults the evidence is weak and inconsistent, while the cholinergic side effects (nausea, bradycardia, sleep disturbance) are real. As a prescription drug it should only be taken under medical supervision.

Donepezil is a reversible, brain-selective acetylcholinesterase inhibitor (AChEI) approved by the FDA and EMA for the symptomatic treatment of mild, moderate and severe Alzheimer's disease. By blocking the enzyme that breaks down acetylcholine, it raises synaptic acetylcholine levels and partially compensates for the loss of cholinergic neurons. Large randomized trials (AD2000, DOMINO-AD) show a modest but real improvement in cognition and daily function, though it does not halt the underlying disease. Its long half-life of roughly 70 hours allows once-daily dosing, usually in the evening.
