WHAT IS DONEPEZIL?
Detailed overview
Donepezil is a reversible, brain-selective acetylcholinesterase inhibitor (AChEI) approved by the FDA and EMA for the symptomatic treatment of mild, moderate and severe Alzheimer's disease. By blocking the enzyme that breaks down acetylcholine, it raises synaptic acetylcholine levels and partially compensates for the loss of cholinergic neurons. Large randomized trials (AD2000, DOMINO-AD) show a modest but real improvement in cognition and daily function, though it does not halt the underlying disease. Its long half-life of roughly 70 hours allows once-daily dosing, usually in the evening.

- Nausea, diarrhea and loss of appetite (the most common, cholinergically driven complaints)
- Sleep disturbance, vivid dreams or nightmares, especially with evening dosing
- Muscle cramps and muscle twitching
- Bradycardia (slow heart rate), fainting or dizziness from increased vagal tone
- Fatigue, headache and weight loss during prolonged treatment

- Known hypersensitivity to donepezil or piperidine derivatives
- Sick sinus syndrome, conduction disorders (SA/AV block) or significant bradycardia
- Active peptic ulcer or high gastrointestinal bleeding risk (increased gastric acid)
- Asthma or obstructive lung disease: cholinergic action can cause bronchoconstriction
- During pregnancy and breastfeeding only after medical evaluation
