WHAT IS HYDERGINE?
Detailed overview
Hydergine is the brand name for ergoloid mesylates (dihydroergotoxine): a mixture of the methanesulfonate salts of four hydrogenated ergot alkaloids. It blocks alpha-adrenergic signaling while acting as a partial agonist at dopamine D2 and serotonin receptors, and it also influences brain energy metabolism and shows antioxidant properties experimentally. Originally introduced as a cerebral vasodilator, the modulation of synaptic neurotransmission is now regarded as its more significant action. A Cochrane meta-analysis of dementia trials found a modest but measurable cognitive effect with good tolerability; it has largely been phased out of practice as newer cholinesterase inhibitors emerged. It is a prescription medicine.

- Nausea and gastrointestinal upset, especially when taken without food
- Nasal congestion and stuffiness
- Lightheadedness, dizziness, orthostatic drop in blood pressure (from alpha-blockade)
- Transient oral mucosa irritation with sublingual dosing
- Bradycardia (slow heart rate) in sensitive patients
- Rare ergot-related fibrosis (retroperitoneal, pulmonary or cardiac valve) with prolonged high-dose use

- Acute or chronic psychosis: ergot alkaloids may aggravate it
- Known ergot alkaloid hypersensitivity or allergy
- Pregnancy and breastfeeding: ergot derivatives should be avoided
- Severe bradycardia or low blood pressure
- Use caution with other ergot derivatives, dopaminergic or serotonergic agents due to additive effects
