WHAT IS VORTIOXETINE?

Vortioxetine (Lu AA21004, marketed as Trintellix/Brintellix) is a prescription multimodal serotonin modulator antidepressant approved for major depressive disorder (MDD) in adults. It simultaneously inhibits the serotonin transporter (SERT) and directly modulates several 5-HT receptor subtypes: it is an agonist at 5-HT1A, a partial agonist at 5-HT1B, and an antagonist at 5-HT3, 5-HT7 and 5-HT1D. This receptor profile secondarily influences the release of noradrenaline, dopamine, acetylcholine, histamine and glutamate in brain regions governing mood and cognition. Beyond mood improvement, clinical trials have shown benefit for the cognitive symptoms of depression (processing speed, executive function), with lower rates of sexual side effects than conventional SSRIs.

  • Nausea, the most common side effect, typically in the first weeks of treatment and gradually easing
  • Diarrhea, constipation or vomiting, usually mild and transient
  • Dizziness, dry mouth and itching
  • Sexual dysfunction, though typically to a lesser degree than with conventional SSRIs
  • Hyponatremia (low blood sodium), especially in the elderly and those on diuretics
  • Rarely serotonin syndrome, especially combined with other serotonergic agents

  • Co-administration with MAO inhibitors is contraindicated due to serotonin syndrome risk (14-day washout required)
  • Known hypersensitivity to vortioxetine or any excipient of the product
  • Extra caution in patients under 25 due to the early risk of suicidal ideation (class effect)
  • History of mania/hypomania or bipolar disorder: an antidepressant may trigger a mood switch
  • Pregnancy and breastfeeding: only after clear benefit-risk assessment under medical supervision
