  • Fine tremor, especially of the hands – the most common β2-agonist effect, dose-dependent and more pronounced with the oral form.
  • Tachycardia and palpitations with rising heart rate; at high doses arrhythmia (e.g. atrial fibrillation) may occur.
  • Hypokalaemia (low serum potassium): β2 activation shifts potassium into cells; can be serious at high doses or combined with diuretics or corticosteroids, and may provoke arrhythmia.
  • Headache, nervousness, anxiety and insomnia – central stimulant effects, milder than with clenbuterol but present despite the short half-life.
  • Muscle cramps (especially calves) – partly linked to hypokalaemia; taurine and magnesium supplementation may ease it.
  • Rapid tolerance (β2-receptor desensitisation): the acute thermogenic and bronchodilator effect fades within 1–2 weeks, encouraging dose escalation.
  • Oral form: more pronounced systemic effects (stronger tremor, tachycardia, insomnia) due to higher plasma levels; nausea may also occur.
  • Inhaled form: local throat irritation, cough, dry mouth and hoarseness; paradoxical bronchospasm is rare but requires immediate discontinuation.

  • Tachyarrhythmia and severe or uncontrolled heart disease (ischaemic heart disease, severe heart failure): β2 stimulation can provoke cardiovascular strain and arrhythmia.
  • Concurrent non-selective beta-blocker therapy: the two mutually antagonise (the beta-blocker abolishes bronchodilation, dangerous in asthma); the combination should be avoided.
  • Uncorrected hypokalaemia: albuterol further lowers serum potassium, raising arrhythmia risk – extra caution with diuretics or corticosteroids.
  • Hyperthyroidism: increased adrenergic sensitivity amplifies tachycardia and tremor.
  • Concurrent MAO inhibitor or tricyclic antidepressant: risk of hypertensive crisis and severe cardiovascular effects.
  • Pregnancy and breastfeeding: only on clear medical indication (asthma); off-label weight-loss use in pregnancy is prohibited.
  • Competitive-sport off-label use: oral/intravenous albuterol is WADA-banned, and even inhaled use is allowed only below 1600 μg/24h (urine <1000 ng/mL) – exceeding it is an anti-doping violation.
