WHAT IS ISOTRETINOIN?

Detailed overview

Isotretinoin (Roche Roaccutane / Accutane, FDA 1982) is the gold-standard treatment for severe nodulocystic acne. 13-cis-retinoic acid (vitamin A metabolite) causes sebaceous gland atrophy, sebum production reduction, and P. acnes growth inhibition. EXTREME TERATOGENICITY (FDA category X) – every female patient must enroll in pregnancy prevention program (USA: iPLEDGE, EU: similar national programs). 2 negative pregnancy tests before start, 2 forms contraception mandatory, monthly testing. Layton 2009 (PMID 20436884) 6-month course produces sustained remission in ~80% of cases. Side effect profile SIGNIFICANT: dryness (mouth, eye, skin, lip), muscle pain, liver function disturbance, depression risk.

Side effects · 7

  • Cheilitis and generalized mucocutaneous dryness in nearly all patients (~90%): chapped lips, dry nose (nosebleeds), dry eyes and skin.
  • Psychiatric effects: low mood, depression, rarely suicidal ideation – an official FDA warning; any mood change must be reported immediately.
  • Blood lipid rise: triglycerides and cholesterol can increase (rare pancreatitis risk), so monthly lipid panel monitoring is mandatory.
  • Hepatic dysfunction: elevated ALT/AST transaminases, so regular liver-enzyme checks are required during the course.
  • Musculoskeletal complaints: muscle and joint pain, back pain, especially with physical exertion; common during treatment.
  • Increased photosensitivity and an initial acne flare (purging) early in the course, plus slower wound healing.
  • Rare but serious: pseudotumor cerebri (benign intracranial hypertension) with persistent headache and visual disturbance, plus reduced night vision.
