WHAT IS NEBIVOLOL (NEBILET, BYSTOLIC)?

Detailed overview

Nebivolol has two actions, and that duality gives it its clinical profile. The first is classical: highly selective beta1-adrenergic blockade, slowing heart rate and reducing cardiac work. Its selectivity is among the highest in the class, which limits unwanted effects on the airways and peripheral vessels. The second action is what sets it apart: nebivolol triggers nitric oxide release in the vascular endothelium and thereby causes vasodilation. The reviews by Maffei 2009 and Münzel 2009 discuss this mechanism in detail. Its practical significance is considerable, because classic beta-blockers do the opposite: through beta2 blockade they cause mild vasoconstriction, contributing to cold extremities, fatigue and erectile dysfunction. With nebivolol the vasodilatory component offsets this, and according to the Weiss 2006 review this explains the more favourable tolerability profile. The backbone of the clinical evidence is SENIORS (Flather 2005), which randomised heart failure patients over 70 to nebivolol or placebo and showed a reduction in the composite endpoint of death or cardiovascular hospitalisation. This matters because earlier beta-blocker trials largely excluded the elderly, often preserved-ejection-fraction heart failure population. One pharmacokinetic peculiarity deserves separate emphasis: nebivolol is cleared by CYP2D6, and that enzyme's activity varies strongly by genotype. In extensive metabolisers bioavailability is about 12%, while in poor metabolisers it can reach 96%. This eightfold difference explains why some people respond unusually strongly to the same dose.

  • Fatigue and dizziness: familiar beta-blocker complaints, but typically milder with nebivolol than with classic agents because of the vasodilatory action.
  • Slowing heart rate: an intrinsic part of the effect, but excessive bradycardia requires dose reduction. A resting pulse below 50 needs medical assessment.
  • Headache and peripheral edema: consequences of the vasodilatory component, usually mild.
  • In slow CYP2D6 metabolisers plasma levels can be many times the usual, so adverse effects are more pronounced. This is roughly 7-10% of people of European ancestry.

Contraindications · 4

  • Decompensated heart failure, cardiogenic shock, second or third degree AV block, sick sinus syndrome and severe bradycardia: absolute contraindications.
  • Severe hepatic impairment: clearance slows substantially, contraindicated.
  • Asthma and severe obstructive lung disease: beta1 selectivity is high but not absolute, so caution or avoidance is warranted.
  • Abrupt discontinuation: beta-blockers must be tapered, as sudden cessation can provoke angina or arrhythmia.
